Özet
Benign paroksismal pozisyonel vertigo, vestibüler rahatsızlıklardan en sık görülenidir. Ayırıcı tanısı ve tedavisi oldukça güçtür. Çalışmamızda baş dönmesi şikâyeti ile kliniğimize başvuran Benign Paroksismal Pozisyonel Vertigo (BPPV) tanısı alan hastaların, Nötrofil/Lenfosit (N/L) ve Platelet/Lenfosit (P/L) hastalık süresince değişimininin incelenmesi amaçlanmıştır. Uygun koşullar taşıyan ve çalışmaya katılmayı kabul eden baş dönmesi şikâyeti ile başvuran ve BPPV tanısı konulan 30 gönüllüye epley ve roll manevraları yapılarak 1. gün ve 10. gün bir tüp kan alınarak N/L ve P/L oranları karşılaştırılmıştır. Kontrol grubu da 22 gönüllü bireyden oluşmuştur. Bireylerin cinsiyet dağılımları gruplara göre incelendiğinde kontrol grubunda 16 erkek 6 kadın bulunurken denek grupta 8 erkek 22 kadın bulunmaktadır. Denek grupta yaş ortalaması 34,5 ±10,1 olup kontrol grupta yaş ortalaması 43,8 ±14,6 dır. Denek grubunun 1.gün N/L oranları ortalaması: 2,63±1,68 10.gün N/L oranları: 2,18±0,84 Kontrol grubunun N/L oranları ortalaması: 2,23±1,54 Aradaki fark BPPV hastalarıyla karşılaştırıldığında istatistiksel olarak anlamlı bulunmuştur. Denek grubunun 1. gün N/L oranları:117,90±28,76 10.gün platelet/lenfosit oranları: 122,90±42,22 Kontrol grubunun P/L oranları:119,68±41,25 Aradaki fark BPPV hastalarıyla karşılaştırıldığında istatistiksel olarak anlamlı bulunmamıştır. BPPV tanı ve tedavisinde pozisyonel manevraların önemli yeri vardır. Bunun yanında BPPV tanı ve tedavisinde bir takım farklı serolojik belirleyicilerin kullanılmasının yeri ile ilgili birçok çalışma yapılmaktadır. Henüz net bir sonuç elde edilememiş ve spesifik bir belirleyici bulunamamıştır. Bizim çalışmamızda da N/L oranındaki düşüş ile BPPV hastalarının iyileşmesi arasında korelasyon olduğu saptanmıştır. Bening Paroxysimal Pozitionel Vertigo (BPPV) is one of the most frequent vestibular disorders. Aim of our study is to evaluate the values of neutrophile/lymphocyte and platelet/lymphocyte ratios in BPPV patients during the disease course. Neutrophile 30 volunteers, accepting the participation, who have consulted with the complaint of dizziness and have been diagnosed with Bening Paroxysimal Pozitionel Vertigo, have been exposed to epley and roll maneuver. On 1st and 10th day, neutrophile/lymphocyte and platelet/lymphocyte ratios have been calculated. Also neutrophile/lymphocyte and platelet/lymphocyte ratios of the the control group which consists of 22 voluntary individuals have been calculated. When the distribution of genders of the individuals is examined due to the gro while there are 16 males and 6 females in the control group, there are 8 males and 22 females in the subject group. In the subject group, age average is 34,5 ±10,1 and the age average is 43,8 ±14,6 in the control group.1st day average of neutrophile/lymphocyte ratio of the subject group is 2,63±1,68 and neutrophile/lymphocyte ratio of the control group is 2,23±1,54. However, a clear result has not been reached yet and a specific determinant could not be found. In our study, too, it has been detected that there is a correlation between the decline in neutrophile/lymphocyte ratio and the recovery of patients with Bening Paroxysmal Positional Vertigo. Further studies are warranted in order to reveal the exact relation between BPPV and the neutrophile/lymphocyte ratio and platelet/lymphocyte ratio.
