Öğe Cardiac Findings on Non-Contrast Thoracic Tomography in Patients with Acromegaly(2022) keskin, lezan; Ilgar, Mehtap; YAPRAK, BÜLENT; Akçiçek, Mehmet; Aslantürk, OkanAim: Acromegaly occurs as a result of excessive and permanent secretion of growth hormone from the pituitary. Mortality is mostly related to cardiovascular system involvement. In our study, we aimed to evaluate the correlation between epicardial fat volume (EFV) and growth hormone level in thorax computed tomography in patients with acromegaly and coronary artery calcification, pulmonary artery diameter, ascending aorta diameter, cardiothoracic ratio (CTO) measurements with the control group patients. Method: Our study was retrospective and included 16 patients with acromegaly who were previously diagnosed and treated by the endocrinology clinic and a control group consisting of 32 patients matched for gender and age.In thorax CT, EFV measurement of the patients was performed and main pulmonary artery diameters, ascending aorta diameters, cardio thoracic ratios, presence of coronary artery calcification were evaluated. Results: The number of patients with large ascending aorta was higher in patients with acromegaly and it was statistically significant (p=0.041). Although the rate of patients with large main pulmonary artery diameter was higher in patients with acromegaly, no significant difference was found between the groups (p=0.355). There was no significant difference between the groups in terms of increased CTO (p=0.818) and coronary artery calcification (p=0.157). Conclusion: In our study, a difference was found between the acromegaly and control group patients only in terms of ascending aorta diameters, but no significant difference was found in terms of other parameters. We think that regular follow-up and treatment of patients is effective in this result. Cardiovascular risks can be reduced in patients with acromegaly with early diagnosis, regular follow- up and treatment.Öğe Effects of sleep quality and insomnia severity on metabolic and anthropometric parameters in elderly individuals(2023) METE, BURAK; keskin, lezanPurpose: This study aimed to investigate the relationship of sleep quality and insomnia with metabolic/anthropometric parameters in elderly individuals. Materials and Methods: This cross-sectional study was conducted with 474 elderly individuals. Their fasting blood glucose, haemoglobin A1c, total cholesterol, triglycerides, low-density lipoprotein, high-density lipoprotein, cortisol, C-reactive protein, weight, waist circumference, waist-to-hip ratio, skeletal muscle mass, body fat mass and body mass index were measured. Their sleep quality was evaluated using the Pittsburgh Sleep Quality Index, and insomnia was assessed using the Insomnia Severity Index. Results: The mean age of the 474 elderly individuals included in the study was 71.02±4.90 years. Poor sleep quality prevalence was found to be 71.1% among the elderly participants and the prevalence of mild to moderate insomnia was 55.1%. The aim of this study was to determine the frequency of sexual dysfunction in patients diagnosed with morbid obesity.Methods: The patient group included in the study consisted of 78 morbidly obese patients with BMI?40 kg/m2 and 68 healthy individuals with normal BMI. The data were obtained by using the sociodemographic information form filled by the participants, Beck anxiety scale, Beck depression scale and Golombok Rust Sexual Satisfaction Inventory.Results: Obese individuals were found to have sexual dysfunction compared to individuals with normal body BMI (p<0.05). When the Golombok Rust subscale scores of obese men were compared to those of normal-weight individuals, a significant deterioration was found in all subscale scores, including frequency, communication, satisfaction, avoidance, touch, impotence and premature ejaculation. In addition, the anxiety and depression scores of obese individuals were shown to be higher than those of normal-weight individuals (p<0.05).Conclusion: Morbidly obese individuals should be evaluated in terms of sexual functions. We believe that regulating obesity treatment, including possible treatment for sexual dysfunction, will increase the success rate and enhance the quality of life for patients.Öğe FARKLI PROTEİN KAYNAKLARI İÇEREN BESİNLERİN KISA SÜRELİ TOKLUK BELİRTEÇLERİ, SERUM İNSÜLİN VE GLUKOZ DÜZEYLERİ ÜZERİNE ETKİSİ(2023) Arslan, Nurgül; ÇAKIROĞLU, FUNDA PINAR; ALATAŞ, HACER; keskin, lezanAmaç: Proteinlerin besin kaynağından dolayı tokluk üzerine etkileri de farklı olabilmektedir. Bu çalışmada iki farklı protein kaynaklı kahvaltının tokluk belirteçleri, serum insülin ve glikoz üzerine etkisini incelemek amaçlandı. Yöntem: Bu çalışma 18-45 yaş arası 31 sağlıklı birey ile randomize çapraz geçişli olarak yürütüldü. Çalışmada farklı protein kaynağına sahip iki farklı kahvaltı bireylere sunuldu. Kahvaltılar bitkisel ve hayvansal kaynaklı besinlerden oluşturuldu. Bireylere kahvaltı sonrasında tokluk durumunu ölçmeyi amaçlayan VAS skalası 30 dakikalık aralıklar ile uygulandı. Bireylerden kahvaltılar sonrası serum insülin ve glikoz seviyesinin ölçümü için 30 dakika aralıklar ile kan numunesi alındı. Bireylerin kahvaltı sonrası ad libitium beslenme ile enerji alım miktarları BEBİS programı ile saptandı ve karşılaştırıldı. Bulgular: İki farklı kahvaltı sonrası bireylerin kombine tokluk skorları arasında 30.,60.,90. ve 210. dakikalardaki farklılıklar istatiksel olarak anlamlı bulundu (p<0.05). Bireylerin serum insülin düzeyleri hayvansal kaynaklı kahvaltıda bitkisel kaynaklı kahvaltıya göre her ölçümde daha yüksek bulundu. Serum glikoz düzeylerinin bitkisel kaynaklı kahvaltıda hayvansal kaynaklı kahvaltıya göre başlangıçta hızlı bir yükseliş görülürken 90. dakikadan sonra hızlı bir düşüşün olduğu saptandı. Ad libitium beslenme sonrası bireylerin bitkisel kaynaklı kahvaltıdan sonra alınan enerjinin ortalamasının daha yüksek olduğu ve her iki kahvaltı arasındaki ortalamalar arasındaki farkın istatiksel olarak anlamlı olduğu saptandı (p<0.05). Sonuç: Hayvansal kaynaklı kahvaltı sonrası bireylerin daha uzun süre kombine tokluk skoruna sahip olduğu, daha yüksek serum insülin düzeyi daha düşük glikoz düzeylerinin olduğu saptandı. Kahvaltı sonrası alınan enerji miktarının hayvansal kaynaklı kahvaltı sonrası daha düşük olduğu saptandı. Çalışma sonuçlarına göre özellikle hayvansal kaynaklı besinlerin daha uzun süre tokluk belirteçlerine neden olduğu sonucuna varıldı.Öğe How effective is the obesity treatment on improving oxidative stress? Is there any difference between drugs?(2022) keskin, lezan; Sahin, ibrahim; Yavuz, Dilek Gogas; Yuksel, Meral; Taskapan, MehmetIntroduction: Obesity shortens the life period and decreases its quality, causing several complications. Recently, oxidative stress produced by lipid peroxydation is considered a cardiovascular risk factor. In this study, we aimed to investigate the relationship between weight loss with lipid profile, insulin resistance, and lipid peroxidation products malondialdehyde (MDA, oxidant) and paraoxonase-1 (PON1, antioxidant) levels which is protective in atherosclerosis, and to evaluate alteration on oxidative stress. Methods: Patients diagnosed as obese at the Endocrinology and Metabolic Diseases Outpatient Clinics of Inonu University Faculty of Medicine between December 2005 and February 2008 were studied. 103 patients were included in the study. Study population was divided into two treatment groups. In the first group, 120 mg of orlistat, three times daily, and in the second group, sibutramine 15 mg per day were given in addition to appropriate diet therapy. The patients were evaluated at the end of three months treatment period. Results: Dramatic changes in body weight, Body Mass Index (BMI), and waist and hip circumference were observed during the three-month evaluation in patients receiving orlistat or sibutramine. Additionally, significant improvements were measured in systolic- diastolic blood pressure levels, lipid levels, and insulin resistance. The decrease in the MDA level and the increase in the PON level were similar and significant in both patient groups. Discussion and Conclusion: The treatment of obesity, aimed at reducing body weight, acts to reduce oxidative stress by increasing paraoxonase-1 activity (antioxidant) and reducing the amount of the peroxidation product malondialdehyde (oxidant), regardless of the type of medication administered.Öğe Postpartum Type 2 Diabetes Mellitus Frequency of Patients with Gestational Diabetes Mellitus(2023) keskin, lezanAim: This study aims to examine the frequency of type 2 diabetes mellitus (DM) in the postpartum period and its relationship with other risk factors. Materials and Methods: Patients who were diagnosed, followed up and treated for gestational diabetes in Malatya Turgut Özal University, Malatya Training and Research Hospital Endocrinology outpatient clinic and who underwent Oral Glucose Tolerance Test (OGTT) in the postpartum period were included. Results: In our retrospective study, 157 patients were included. The mean age was 31.8±5.6 years. After being followed up with her follow-up and treatment throughout the pregnancy, OGTT administered with 75 grams of glucose was performed at the postpartum 8th week. Impaired glucose tolerance (IGT) was detected in 23 patients (14.64%), impaired fasting glucose (IFG) in 18 patients (11.46%), and type 2 DM in 17 patients (10.8%). When evaluated in terms of the presence of DM in the postpartum period, no difference was found regarding the history of gestational DM in the anamnesis (p=0.305) and the presence or absence of family history (p=0.095). In terms of the presence of DM, there was a significant difference between the patients receiving insulin therapy and those receiving diet therapy (p=0.001). Conclusion: We can say that type 2 DM development in the postpartum period is associated with high maternal age and an increase in body mass index (BMI). Being pregnant with the ideal weight to be achieved by lifestyle changes may decrease the risk of type 2 DM and diabetes-related complications in the long term.Öğe The investigation of vestibulary system in patients with thyroid dysfunctions(2024) koca, çiğdem fırat; keskin, lezan; KELLEŞ, Mehmet; ÇELİK, TURGUT; Aydin, Sukru; YAŞAR, ŞeymaA relationship has been detected among thyroid dysfunctions and vestibular pathologies. Our aim was to evaluate the vestibular system in cases with thyroid dysfunctions. Totally 71 patients between the ages of 20-50 with thyroid dysfunctions (35 hypothyroidic and 36 hyperthyroidic) and 36 participants with normal thyroid functions were included. Patients were also allocated as two groups based on antibody positivity. Pure tone audiometry and Vestibular Evoked Myogenic Potential tests were performed for all participants. In the left ear there was a numerically significance in the hypothyroids at 4000 and 8000 Hz (p=0.020 and p=0.014). In the right ear p13 and n23 latencies differed among the groups, (p=0.004 and p=0.001). The p13-n23 and n10-p15 amplitudes were decreased in both patient groups (p<0.001) (p=0.015). For the left ear, the p13 an n23 latencies was prolonged in both patient groups (p=0.044) (p=0.002). The p13-n23 amplitude value was distincted numerically significant (p<0.001). The n10-p15 amplitude was longer in the patients (p=0.004). We can not observe a numerically significance among groups according to antibody positivity (p>0.005). Thyroid dysfunctions may lead to subclinical changes on the vestibular system and the saccular and utriculer impairment may be an early indicator of endolymphatic hydrops.Öğe The Role of Hedonic Hunger As a Moderator and Mediator in Older Adults Obesity(2024) METE, BURAK; keskin, lezan; Demirhindi, Hakan; Şahin, Ceren Kanat; METE, ESRA DOĞAN; Bayram, ErtuğrulObjective: In the modern age, the pursuit of food is now a means of pleasure and of satisfying hunger. This study examined the role of hedonic hunger and eating behaviors on obesity in older adults. Materials and Methods: This cross-sectional study was conducted on 400 older adults. The nutrition information system (BeBiS), bioelectrical impedance analysis, power of food scale-Turkish version (PFS-Tr), and Dutch eating behaviour questionnaire (DEBQ) were used to evaluate the subjects. Results: The mean age of 400 people included in the study was 67.82±2.77 years. It was found that the moderator effect of the interaction of daily energy intake and hedonic hunger behaviour (PFS-Tr) on the waist-to-hip ratio was significant. In particular, the regulatory effect of low hedonic hunger scores (PFS-Tr) was significant (p=0.019) and the waist-to-hip ratio decreased as the scores obtained from the scale decreased. The mediator effect of the restrained eating (ResE) sub-dimension (DEBQ) between the average daily energy intake and body fat mass was significant (estimate =0.221, p=0.021), and an increase in ResE behavior scores resulted in an increase in body fat mass. It was found that 89.4% of the effect of ResE behaviour on body fat mass was direct and large. There was a weak positive correlation between the average daily energy intake and food available subscale of PFS-Tr (r=0.157) and food present subscale (r=0.017). Total daily energy intake had a direct effect on body fat mass (estimate =0.008, p=0.005). Conclusion: The results of our study support a positive relationship between hedonic hunger and abdominal obesity in older adults. Decreased ResE behaviour increases the whole-body fat mass. Evaluation of hunger and implementation of programmes that support ResE behaviour may be useful in controlling obesity and its negative consequences in older adults.
