    Acil Serviste Miyokard Enfarktüsünde Hemogram Parametrelerinin Tanısal Değeri

    Amaç: Bu çalışmada, acil servise göğüs ağrısı ile başvuran hastalarda miyokard enfarktüsünün tanısında lökosit sayısı, lökosit-trombosit oranı ve nötrofil-trombosit oranı gibi hemogram temelli inflamatuvar parametrelerin tanısal değerinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Ocak–Şubat 2025 tarihleri arasında acil servise göğüs ağrısı ile başvuran, 18 yaş üzeri 60 miyokard enfarktüslü ve 60 miyokard enfarktüsü olmayan hasta retrospektif olarak incelendi. Hemogram, biyokimya, elektrokardiyografi ve kreatin kinaz-MB değerleri değerlendirildi. Lökosit sayısı, lökosit-trombosit oranı ve nötrofil-trombosit oranı iki grup arasında karşılaştırıldı. Tanısal performans alıcı işletim karakteristiği analizi ile değerlendirildi ve optimal eşik değerler Youden indeksi kullanılarak belirlendi. Bulgular: Glukoz, kreatin kinaz-MB, lökosit sayısı, nötrofil sayısı, lökosit-trombosit oranı ve nötrofil-trombosit oranı miyokard enfarktüsü grubunda kontrol grubuna göre anlamlı derecede daha yüksek bulundu (p<0.001). Alıcı işletim karakteristiği analizinde, lökosit-trombosit oranı en yüksek tanısal doğruluğa sahip olup eğri altında kalan alan değeri 0.732 olarak saptandı. Bu parametre için kesim değeri 0.0365 olup duyarlılık %71.7 ve özgüllük %66.7 idi. Sonuç: Hızlı, düşük maliyetli ve kolay erişilebilir hemogram parametreleri miyokard enfarktüsünün tanısında anlamlı katkı sağlayabilir. Özellikle lökosit-trombosit oranı, troponin sonuçları beklenirken klinik karar verme sürecini destekleyebilecek pratik bir biyobelirteç olabilir.

    An evaluation of the relationship between subjective tinnitus perception and COVID-19-related psychological factors

    Aim: This study aimed to examine the relationship of tinnitus loudness, annoyance and handicap in tinnitus patients with the level of coronavirus-related anxiety, psychological distress, and fear. It was also aimed to evaluate the effects of insomnia severity and social and emotional loneliness perception on tinnitus in the pandemic period. Materials and Methods: A total of 112 patients over the age of 18 who were followed up in two centers with the diagnosis of chronic subjective tinnitus were included in the study. During the pandemic, the data were collected using the following tools via Google Forms: Demographic Information Form, Visual Analogue Scale (VAS) for tinnitus loudness and annoyance, the Tinnitus Handicap Inventory (THI), Coronavirus Anxiety Scale (CAS), Fear of COVID Scale (FCVS-T), COVID-19 related Psychological Distress Scale (CORPD), Insomnia Severity Index (ISI), and Social and Emotional Loneliness Scale (SELSA-S). Results: A weak positive correlation was found between CAS score and VAS for tinnitus loudness and annoyance, FCVS-T, ISI, Selsa-S total scores; between FCVS-T score and VAS for annoyance, CORPD, ISI scores; and, between CORPD and THI, ISI scores (p<0.05). There was a significant positive correlation between VAS and Selsa-S \"social loneliness\" sub-dimension scale, THI scores (p<0.05). Conclusion: Psychological support should not be neglected in the management of tinnitus patients in the pandemic period, and tinnitus patients should be followed closely, since the increase in psychological factors and the perceived loneliness level and the severity of insomnia in the pandemic cause a worsening in the perception of tinnitus.

    Assessing lipoxin-mediated inflammatory responses in the second trimester of pregnancy among women with obesity: A comprehensive analysis

    Objective: This study aimed to explore the relationship between maternal plasma lipoxin A4 (LXA4) levels during the second trimester of pregnancy and certain proinflammatory molecules, such as interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-?), as well as the antiangiogenic factor vascular endothelial growth factor receptor 1 (VEGFR-1), in conjunction with obesity among pregnant women. Materials and Methods: A total of 30 pregnant women with obesity were compared with 30 pregnant women of normal weight, matched for both age and gestational week. Plasma samples were collected from all participants between the 18th and 28th weeks of pregnancy. The levels of LXA4, VEGFR-1, IL-6, and TNF-? were quantified using enzyme-linked immunosorbent assay. Results: Plasma levels of LXA4 were notably lower in pregnant women with obesity, whereas levels of TNF-? and VEGFR1 were significantly higher (p=0.041, p<0.001, and p<0.001, respectively). There was no significant difference in IL-6 levels between groups (p=0.072). The binary logistic regression model revealed significant associations between obesity and the examined inflammatory mediators. Specifically, the results demonstrated that higher levels of LXA4 were linked to a reduced obesity risk, with each unit increase corresponding to a 0.926-fold decrease in the likelihood of obesity. Conversely, elevated levels of TNF-? and VEGFR1 were associated with an increased risk of obesity. Conclusion: The study concluded that increased body mass index during pregnancy affects the levels of plasma lipoxin, cytokines, and angiogenesis-related factors. Although the exact mechanisms remain unclear, the observed changes suggest a disruption in the metabolic systems of women with obesity, which may influence physiological changes during pregnancy and lead to obesity-related pathological conditions.

    Clinical Comparison of Patients Undergoing Anterior Cruciate Ligament Reconstruction Using an Allograft or Autograft

    Aim: The purpose of this study was to examine the clinical results of individuals who underwent an autograft or allograft repair of the anterior cruciate ligament (ACL). Material and Methods: Retrospective analysis was done on the patient files of patients who underwent ACL reconstruction between 2014 and 2020 using semitendinosus-gracilis tendon autografts (SGT-A) and tibialis anterior tendon allografts (TAT-A). In this study, the data of 30 patients in each group were included. Knee laxity tests, the Lysholm knee grading system, the Tegner activity score, and the International Knee Documentation Committee (IKDC) score were used to compare patient results. Results: While there was no significant difference in the Tegner activity score between the preoperative and the final measurement (p=0.241), the IKDC scores and the Lysholm knee ratings changed statistically significantly between the preoperative measurement and the last control visit (p=0.020, and p=0.038, respectively) for both groups in this study. The SGT-A group’s Lysholm knee score had a preoperative value of 60.97% and a final control value of 90.48%. The preoperative Lysholm knee score for the TAT-A group was 61.31%, and the final control value was 95.03%. The anterior drawer and Lachman test findings showed statistically significant intergroup (autograft and allograft) alterations in both the autograft and allograft groups (both p<0.001). Conclusion: In terms of knee function and laxity, this study achieved a better clinical outcome in the allograft group compared to the autograft group.

    Comparative analysis of models in confirmatory factor analysis: Exploring clinical applications and interpretation

    To demonstrate the explainability of the scales with fewer dimensions instead of the number of existing dimensions by ensuring that the scale structures created by explanatory factor analysis (EFA) are verified with confirmatory factor analysis (CFA). Data from the Nutritional Behavior Scale in Children, answered by the parents of 204 children with autism spectrum disorder (ASD) were used. EFA was performed with the data obtained from the scale. In the next step, the explained variance percentages and dimensions were determined and the model goodness of fit indexes were calculated with CFA. The dimensions with the lowest explained variance percentages were removed from the model, respectively, and three different scale models were tested. The variance explanation percentage of the first eight-dimensional model created with EFA was calculated as 72.68%. The food fussiness sub-dimension was removed and CFA was applied to the model again and new indices were calculated. Finally, the emotional under-eating sub-dimension was excluded from the model, resulting in a six-dimensional Child Eating Behavior Scale (CEBS). Goodness-of-fit indices of the CFA model established with six dimensions were ?2 / df; 1.545, AIC; 715,433 and RMSEA; 0.052 was found. It has been shown that the eight, seven, and six-dimensional scale models constructed according to the percentages of variance explained for CEBS are sufficient to explain the sample and that the six-dimensional scale model can be used for CEBS. Our study is the first to use competing models in confirmatory factor analysis in reducing scale dimensions.

    Could ELABELA be a Protective Biomarker in Patients with Abnormal Uterine Bleeding?

    Aim: Abnormal uterine bleeding (AUB) is a health problem characterized by various symptoms such as heavy and prolonged menstrual bleeding, affecting approximately 30% of female patients both physiologically and psychologically. The objective of this study was to assess serum Elabela (ELA) concentrations in women aged 18 and above diagnosed with functional AUB, and to compare these concentrations with those of healthy women. Material and Method: This prospective case-control study was performed from August 18, 2022 to December 30, 2022. This was a cross-sectional study including 50 women who applied to the gynecology service of Malatya Turgut Özal Training and Research Hospital with complaints of AUB and 50 women without AUB who underwent gynecological examination. The presence of AUB in patients was determined based on clinical examination conducted by a gynecologist and medical records. Demographic and clinical characteristics were recorded. Serum ELA levels were determined by commercial ELISA kit. Results: Serum ELA levels was significantly lower in patients with AUB (581.54±272.25 pg/mL) compared to the healthy group (744.55±300.31 pg/mL, p=0.005). In this study, ELA in patients with AUB showed 98% sensitivity and 80% specificity with a cut off value of 411.41 pg/mL (area under the curve [AUC], 68.1%; p=0.002). Conclusion: Serum ELA levels in patients with AUB were significantly lower than in healthy women. These results show that ELA is a good predictor of the pathophysiological process of AUB.

    Cross-cultural adaptation, reliability, and validity of the Turkish version of moral courage scale for physicians

    Aim Physicians, like other healthcare professionals, frequently encounter situations requiring moral courage in their professional lives. However, studies on the moral courage of physicians are limited in the literature. This study aims to evaluate the Turkish adaptation, validity, and reliability of the Moral Courage Scale for Physicians (MCSP) and to contribute to national and international studies on moral courage among physicians. Materials and methods This methodological study involved the adaptation, validation, and reliability assessment of the MCSP in Turkish. The scale’s internal consistency was evaluated using Cronbach’s α coefficient, and the total correlation coefficients for the items were calculated. Test-retest reliability was assessed using a two-group design. Results The Turkish version of the MCSP demonstrated high internal consistency, with a Cronbach’s α coefficient of 0.91. The total correlation coefficients for the items ranged from 0.387 to 0.797. A significant positive correlation was found between the test and retest scores of the MCSP (p < 0.05). Conclusion The Turkish version of the MCSP is a valid and reliable tool for assessing moral courage among physicians. © 2025 Yılmaz et al.     CRP and LDH Levels Can Be Used for Support the COVID-19 Diagnose in Intensive Care Unit Patients

    Aim: The coronavirus disease (COVID-19) has been a public health problem that causes severe acute respiratory syndrome affected all over the word since 2019. The most commonly used parameters as inflammatory response in the clinic are leukocytes, neutrophils, erythrocyte amount and serum C-reactive protein (CRP). In recent years, it has been reported that serum PCT (procalcitonin) level may be useful in the diagnosis of bacterial and viral infections. The aim of our study is to compare blood parameters that may play a supportive role to diagnose of COVID-19 in healthy control and critically COVID-19 patient groups. Material and Methods: This retrospective research was carried out in Malatya Turgut Ozal University Training and Research Hospital, Malatya, Türkiye. Total 88 critically ill patients and 90 healthy people accepted to the study and electronic medical records of patients and control group has been collected from hospital information system (HIS). COVID-19 diagnose has been confirmed by real-time polymerase chain reaction (RT-PCR) results. Results: No statistically significant difference was found between the patient and control groups according to gender in the participants included in the study. A statistically significant increase was observed in CRP, LDH, PCT, D-dimer, urea, sediment, lympocyte and neutrophil levels in COVID-19 patients. According to logistic regression analysis CRP, LDH and sediment values were found to be statistically effective in estimating the COVID-19 infection. These results also supported by ROC analysis, CRP, neutrophil, LDH, PCT and D-dimer results were determined to be distinguishing parameters for COVID-19 patients. Conclusion: We found that CRP, PCT and LDH levels higher in the COVID-19 patients and these parameters can be used to diagnose and estimate the prognose of COVID-19 infection in intensive care patients.

    Effect of post-acute COVID-19 on motor performance and attention in children

    Study was conducted to investigate motor performance and attention levels of children who had experienced post-acute coronavirus disease 2019 (COVID-19) infection. The study included children aged 6-14 years who were diagnosed with post-acute COVID-19 at least four weeks prior and had no history of orthopedic or neurological conditions (Group I). A control group (Group II), composed of children in the same age range, had never been diagnosed with COVID-19 and had no familial history of the virus, nor any orthopedic or neurological disorders. Demographic information for both groups was collected using a descriptive questionnaire. Motor performance was assessed through  Timed Up and Go Test (TUG), Stair Climbing Test (SCT), while attention was measured using Conners' Parent Rating Scale-Revised, Short Form (CPRS-R:S). Difference in the CPRS-R:S scores of the participants based on their post-acute COVID-19 infection status was found statistically significant (p0.05). Accordingly, children who recover from post-acute COVID-19 would have a CPRS-R:S score 0.955 times as high as the score of those who have not had COVID-19 (OR= 0.955, 95% CI 0.919-0.993). The attention levels of children diagnosed with post-acute COVID-19 are affected negatively, considering relationship between attention and motor performance, it is needed to assess children holistically.

    Effects of different measurements of foot angle on vital activities of hemodialysis patients

    Aim: Individuals who receive hemodialysis treatment due to chronic renal failure have limitations in their daily lives and a decrease in physical activity and walking speed. In this study, we have investigated the effectiveness of various measurements of ankle angles of hemodialysis performed on patients on balance, falling risk, and daily life activities. Materials and Methods: 114 volunteer hemodialysis patients with a mean age of 30-94 have measured at the position of plantar flexion, inversion, and eversion by a 1-degree sensitivity manual goniometer. The Time Up and Go test (TUG), Lawton and Brody Instrumental Activities of Daily Living (IADL) Scale and Barthel Index (BI) were applied to the patients. Statistical analyses were made with the SPSS 25 program. Results: 55 male and 59 female participants were accompanied in the study. The average age was 62.8 ± 15.29, the average hemodialysis duration was 5.59 ± 3.92, the average body mass index was 5.08 ± 5.1 and he average number of falls was 5.59 ± 3.92. A low positive significant correlation was observed between plantar flexion and inversion foot angle measurements with The Lawton and Brody IADL scale (p<0,05). A low-level significant positive correlation was observed between the BI score and the plantar flexion angle measurements (p<0.05). A highly significant positive correlation was observed between plantar flexion angle measurement and inversion angle measurement. In contrast, a moderately significant positive correlation was observed between plantar flexion angle measurement and eversion angle measurement (p<0.05). Conclusion: We observed a decrease in daily life activities and a walking speed increase in falling risk in the diagnosis of chronic renal failure in hemodialysis patients.

    Effects of the february 6 earthquake on the agents growing in urine cultures and antibiotic resistance rates

    OBJECTIVE: The aim of this study was to investigate the effects of the February 6 earthquake on the bacteria grown in urine cultures and antibiotic resistance of patients admitted to the urology outpatient clinic. METHODS: In this study, 11,397 urine samples requested by the urology outpatient clinic between January 2022 and De- cember 2023 were examined. We evaluated the urine cultures and antibiotic resistance profiles of the pathogens in patients’ clinic in the years 2022 and 2023 separately and post-earthquake year 2023 as two separate periods, as the first 6-month period immediately after the earthquake. The pathogens grown in urine cultures and antibiotic resistance profiles were ex- amined retrospectively. Only one of the same strains isolated in consecutive repeated growths in the urine cultures of the patients was included. RESULTS: Of the 11,397 urine cultures, 6,914 were requested in 2022 and 4,483 were requested in 2023. There was no statistical difference in the growth rate between the years (13.1–14.1%). When the demographic data of the first and last 6 months of 2023 were compared, statistical differences were found between the first and last six months of 2023 in terms of the number of patients for whom culture was requested, the number of culture positives, the reproduction rate, and the number of male and female patients. The total number of patients for whom urine culture was requested was 4483 (1471 in the first six months - 3012 in the last six months) (p=0.008) and the number of culture-positive patients was 630 (184–446) (p=0.007). The reproduction rates were found to be 12.5% and 14.8%, respectively (p=0.047). It was found that the resistance rates of Escherichia coli (E. coli) to gentamicin (GN), cefuroxime (CXM), cefixime (CFM) and ceftazidime (CAZ) groups of antibiotics were higher in the first six-month period of 2023 (p<0.05). Similarly, the re - sistance rate of Klebsiella spp. to CXM and CAZ groups of antibiotics was found to be higher in the first six-month period of 2023 (p<0.05). CONCLUSION: The results of our study show that the conditions in communal living areas after the February 6 earthquake had an increasing impact on antibiotic resistance rates and antibiotic resistance rates of certain groups of antibiotics, espe- cially the Enterobacterales group were higher in the first half of 2023.

    Evaluation of post-earthquake fractures according to anatomical body parts: A retrospective study

    Bone fractures that occur in earthquakes threaten human life. The study aims to localize the fractures in earthquake victims according to anatomical regions in the earthquake centered in Kahramanmaraş 2023. The study included 137 patients aged between 1-90 years diagnosed with fractures by an orthopedic physician. Radiographs were taken of patients admitted to the emergency department after the earthquake. The fractures diagnosed patients were evaluated and classified in anatomical regions and fracture type. The fractures were observed in females with high rates of fragmented (49.6%) and s ingle fractures (75.9%). The fractures also occurred on the right side (58.4%), in the upper extremities (37.2%) and long bones (38.7%). In the specific anatomic region evaluation of fractures; high rates were observed in manus (19%), antebrachi (15.3%), crus (13.9%) and pes fractures (12.4%). In the region of the most common fragmentary fracture type (48.9%); omos (3%), brachium (1%), thorax (15%), viscerocranium (7%) and neurocranium (3%), in oblique fractures (25.5%); pes (26%), in transverse fractures (20.4%); antebrachi (21%), manus (32%) and vertebral column (4%), in spiral fractures; pelvis (14%), femur (14%) and crus (57%) region. We determined that fractures frequently occur on the right-side extremities and, besides sequentially high rates at manus, antebrachii, crus, and pes. We recommend providing information about the protection of these anatomical parts in the training given to people during the earthquake closure and post-earthquake panic times to reduce earthquake-related fractures.

    Evaluation of the efferent auditory system in COVID-19 adult patients

    Background: The short- and long-term effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection on the medial olivocochlear reflex and outer hair cells in the cochlea remain largely unclear. Aims: The aim of this study was to investigate the efferent auditory system effects in adult patients with COVID-19. Materials and methods: The study included 18–50 years old 44 volunteers: 26 individuals (52 ears) with COVID-19 in the study group and 18 healthy individuals (36 ears) in the control group. Otolaryngological examination, immitancemetric evaluation, distortion product otoacoustic emission (DPOAE), contralateral acoustic stimulation with DPOAE, audiometric evaluation, and high frequency audiometric evaluation were performed in all individuals participating in the study. Results: In our study, patients with COVID-19 had significantly lower DPOAE results with or without broadband noise at only 6 kHz frequency and contralateral suppression results at all frequencies compared to healthy individuals. A statistically significant difference was found between the study and control groups according to whether the participants had a response in the high frequency audiometry at 12 and 16 kHz frequencies. Conclusions: COVID-19 affects many systems in the body. As a result of the findings obtained in the present study, it is shown that the auditory efferent system may also be affected.

    Evaluation of volume measurements of neuroanatomical structures related to speech in multiple sclerosis patients

    Background/aim: Individuals with multiple sclerosis (MS) may experience various speech-related issues, including decreased speech rate, increased pauses, and changes in speech rhythms. The purpose of this study was to compare the volumes of speech-related neuroanatomical structures in MS patients with those in a control group. Materials and methods: The research was conducted in the Neurology and Radiology Departments of Malatya Training and Research Hospital. The records of patients who presented to the Neurology Department between 2019 and 2022 were examined. The study included the magnetic resonance imaging (MRI) findings of 100 individuals, with 50 in the control group and 50 patients with MS, who had applied to the hospital in the specified years. VolBrain is a free system that works automatically over the internet (http://volbrain. upv.es/), enabling the measurement of brain volumes without human interaction. The acquired images were analyzed using the VolBrain program. Results: As a result of our research, a significant decrease was found in the volume of 18 of 26 speech-related regions in MS patients. It was determined that whole brain volumes decreased in the MS group compared to the control group. Conclusion: In our study, volume measurements of more speech-related areas were performed, unlike the few related studies previously conducted. We observed significant atrophy findings in the speech-related areas of the frontal, temporal, and parietal lobes of MS patients.

    EVLİ ÇİFTLERDE 2D:4D PARMAK UZUNLUK ORANININ KİŞİLİK ÖZELLİKLERİ, EVLİLİKTE DOYUM VE UYUM ÖZELLİKLERİ ARASINDAKİ İLİŞKİYE ETKİSİNİN İNCELENMESİ

    Evlilik uyum ve doyumları yüksek olan bireylerin yaşam doyumları, hayat kaliteleri de yüksek düzeydedir. Bireylerin kişisel özellikleri hayatın her alanında etkili olduğu gibi evlilikler üzerinde de etkilidir. 505 evli bireyin katılımı ile gerçekleştirilen çalışmada ilişkisel tarama modeli uygulanmıştır. Sağ ve sol el 2D:4D parmak uzunlukları oranları, “Beş Faktörlü Kişilik Envanteri”, “Evlilik Doyumu” ve “Evlilik Uyumu” ölçekleri kullanılarak yapısal eşitlik modellemesi diyagramı kurulmuştur. Sağ ve sol 2D:4D parmak uzunluklarının ölçekler üzerinde etkisinin olmadığı bulunmuştur. Modifikasyonlar ile kurulan ikinci modelde kişilik özelliklerinin, evlilik uyumu (p=0.001<0.05), evlilik uyumunun dadoyum (p=0.001<0.05) üzerindeki etkisi istatistiksel olarak anlamlı bulunmuştur. Bireylerin kişisel özelliklerinin evlilik uyumlarını, evlilik uyumlarının da evlilik doyumlarını etkilediği bulunmuştur. Çalışmamız hem sağ ve sol 2D:4D parmak uzunlukları oranlarının evlilik ile ilişkisini incelemesi hem de ölçekler arasındaki ilişkinin analizinde Yapısal Eşitlik Modellemesi kullanması açısından literatürdeki ilk çalışmadır.

    Exploring Irisin and Nesfatin-1 in second trimester amniotic fluid: a comparative study of obese and normal weight pregnant women

    Objective: Obesity is characterized by dysregulated adipokine production patterns, leading to low-grade inflammation. Irisin has been identified as a potential target for the treatment of diabetes and obesity. Nesfatin-1, another adipokine, plays a significant role in various metabolic processes, including glucose homeostasis. This study investigated the levels of the new generation adipokines, irisin and nesfatin-1, in the amniotic fluid during the second trimester of normal and obese pregnancies. Methods: Amniotic fluid samples were collected following established protocols. The first 2 mL portion of fluid obtained during amniocentesis was retained aAfter centrifugation at 1500 g for 10 minutes, the cell-free amniotic fluid was transferred to Eppendorf tubes and quantitative measurements of irisin and nesfatin-1 levels were performed using the Enzyme-Linked Immuno Sorbent Assay (ELISA) method. Results: Our results demonstrate, for the first time in the literature, the presence of irisin and nesfatin-1 in amniotic fluid. Additionally, we found that the levels of these adipokines were significantly lower in obese pregnant women compared to the control group (both p<0.05). Conclusion: Our results demonstrate, for the first time in the literature, the presence of irisin and nesfatin-1 in amniotic fluid.     Hekimlerin Klinik Kaliteye Bakış Açısını Ölçmeye Yönelik Bir Ölçek Geliştirme: Metodolojik Çalışma

    Amaç: Bu çalışma ile hekimlerin klinik kaliteye yönelik bilgi ve algılarının değerlendirmesi için geçerli ve güvenilir bir veri toplama aracının geliştirilmesi ve bu sayede hekimlerin Türkiye’deki klinik kalite ile ilgili bilgi ve algılarının ölçülmesi ile klinik kalite süreçlerinin iyileştirilmesine katkı yapması hedeflenmiştir. Gereç ve Yöntemler: Veri toplama süreci sonunda hazırlanan verilere öncelikle Açıklayıcı faktör analizi (AFA) uygulamak için SPSS 25.0 (Statistical Program in Social Sciences) programı kullanılmıştır. Yapılan AFA sonucunda elde edilen ölçeğe ait faktör yapısının doğruluğunu test etmek için AMOS 24 kullanılmış ve doğrulayıcı faktör analizi (DFA) yapılmıştır. DFA sonucunda kurulan ölçeğe ait modelin uyum iyiliği test değerleri yorumlanmış ve ölçeğin son hali kullanıma hazır hale getirilmiştir. Analizlerde istatistiksel olarak güvenilirlik katsayısı 0,95 ve anlamlılık seviyesi ise 0,05 olarak alınmış ve hesaplamalar yapılmıştır. Bulgular: Araştırmada kullanılan 20 maddelik Klinik Kalite Ölçeğinin yapı geçerliliği için uygulanan AFA sonucunda faktör yükü değerleri; 0,536 ile 0,908 arasında bulunmuştur. Toplam varyansın açıklama yüzdesi ise %65,145 olarak hesaplanmıştır. Klinik Kalite Ölçeğine ait iç tutarlılık katsayısını hesaplamak için Cronbach ? güvenilirlik analizi uygulanmış ve Cronbach ? değeri yaklaşık 0,97 olarak hesaplanmıştır. Sonuç: Yapılan çalışma neticesinde, “Klinik Kalite Ölçeği” geçerli, güvenilir bir ölçme aracı olduğu sonucuna ulaşılmıştır.

    Investigation of the Effects of Smartphone Use on the Dominant Thumb and Wrist of University Students

    Aim: In this study, the effects of the addiction level of smartphone use in university students on the range of motion and proprioception of the dominant hand, wrist range of motion (ROM), and thumb ROM were investigated. Material and Methods: Wrist and thumb ROMs were measured with a manual goniometer with a sensitivity of 1 degree, and hand grip strength was measured with a hand grip dynamometer in a total of 100 volunteer university students with a mean age of 18-25. Smartphone Addiction Scale-Short Form (SAS-SF) and Patient Rated Wrist and Hand Evaluation (PRWHE) questionnaire were applied to the students. Statistical analyzes were made using the SPSS 25 program. Results: The students’ usage time of smartphones and addiction levels do not affect the hand grip strength; the increase in smartphone addiction statistically correlates with the right thumb flexion (0.016) and abduction ROMs (0.015), statistically correlated with increased pain level in daily life and statistically correlated with the decrease of the wrist radial deviation ROM (0.009). As the duration of smartphone use increases, the error rate in right thumb abduction proprioception statistically increases (0.027). In addition, we determined the statistically correlations in both thumb flexion and abduction movements. Conclusion: We determined that the excessive usage of smartphones affects the ROM of thumb flexion and abduction, the ROM of wrist radial deviation, and the proprioception of thumb abduction, and it does not affect the hand grip strength. The findings of our research will be a source for future studies.

    Iron Deficiency may be a Risk Factor for Inguinal Hernia Development in Children

    Objectives: This case control study aimed to investigate whether the routine hemogram and biochemical parameters of pediatric patients who have undergone surgery for inguinal hernia are associated with the etiopathogenesis of the disease. Methods: Eighty cases of inguinal hernia surgery performed between January 2019 and November 2022 were included in the study. A control group was also established using hospital records, consisting of eighty pediatric patients without any known his- tory of hematological or metabolic disease or use of regular medication. Statistical analysis was conducted to compare the total hemoglobin (Hgb), hematocrit (Htc), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), erythrocyte distribution width (RDW) and thrombocyte (PLT) values in both groups. Results: The age range of the pediatric patients was 1-14 years. Of the eighty children, 47 (58.8%) were male and 33 (41.3%) were female, with a mean age of 5.79±3.26. The values of Hgb, Htc, MCH, MCHC, and MCV in the inguinal hernia patients were found to be statistically significantly lower than those in the control group (p<0.05). Additionally, the patient RDW values were found to be statistically significantly higher than those in the control group (p<0.05). Conclusion: Compared to the control group, the observed decrease in MCH, MCHC, MCV, Hgb, HTC values, as well as the increase in RDW in patient group, suggests a predisposing effect of iron deficiency. These specific changes suggested that iron deficiency may lead structural changes in the collagen construction and may contribute the etiopathogenesis of childhood inguinal hernia.
